What does one do when they achieve the feeling of utter hopelesness, to a degree where they do not consider themselves "themself" anymore?

What does one do when it feels as if an entire world of misfortune and setbacks curses said "one" for a time that feels like eternity, in a manner so utterly devastating that you cannot put into words how it feels?
